Retirement and the Transition of Care

It is with a mix of sadness and gratitude that we announce Dr. Stacia Poole's retirement as of November 30, 2020. The office is now officially closed. We want to ensure a smooth transition for your ongoing care and have updated our website with details to assist you during this time. Requesting Medical Records and the Process

We understand that requesting and transferring medical records can be a complex process. Please allow up to 30 days for us to complete the preparation and transfer of your records. We manually compile the various parts of your electronic chart into a new document for release. If you have a scheduled visit with another doctor within two weeks, please let us know, and we will make every effort to expedite your request. Medical records requests will be coordinated through Dr. Jeannine Stein's office. A signed release form is required, which can be any doctor's form or downloaded from our website, and can be mailed, faxed, or delivered to her office at 7960 N.
