Patient Information
To begin the treatment process, we will need new patients to fill out a "New Patient Information Form" that covers their medical history, current condition, allergies, insurance, and any other pertinent factors we need to know about. For younger patients, a parent/guardian can fill out the form before the appointment. After an examination, we will be able to identify any early warning signs of issues and recommend the most effective treatment. If the patient needs braces or wants to consider another form of teeth straightening treatment, we will consult with the parent/guardian on the possible options.
