Health Care Foundation

At the Healthcare Foundation of Northern Sonoma County, we believe that every person deserves access to good healthcare and mental healthcare, right here at home. Our mission is to connect people and resources to promote wellness in underserved communities. We invest in programs that support equitable access to culturally-sensitive healthcare and mental health services, and promote overall community wellness.

Our Guiding Principles

Our guiding principles inspire our actions, including grantmaking, board membership and hiring, strategies, and interactions with one another. These principles are rooted in community, compassion, and equity. We believe that everyone deserves to be treated with dignity and respect, and that healthcare is a fundamental human right.

Our Strategy

We support a range of programs and initiatives that promote health education and professional development, mental health talent pipeline projects, community health workers and promotores, hope SRJC scholarships, community resilience engagement and support, community wellness convenings, community response teams, and emergency COVID-19 funding. Our strategy is focused on eliminating health inequities in northern Sonoma County, and we work closely with our community partners to achieve this goal.

Our Impact

We are proud of the impact we have made in northern Sonoma County. To date, we have awarded over $24.5 million in grants since 2001, and have supported over 100 bilingual, bicultural students through our scholarships. We have also established a mental health talent pipeline project, which has helped to address the shortage of mental health professionals in our region.

Our team is dedicated to promoting health equity in northern Sonoma County. Our board and staff are committed to our mission and vision, and work tirelessly to support our community partners and grantees.
