About Your Visit

For your appointment, please arrive early to complete the registration process. Bring your driver's license or state ID, referral if required by your insurer, insurance information, copies of previous medical records, and a list of current medications and any known allergies. We participate with most major insurance plans and make every effort to bill your insurance for you. Payment is due at the time of service unless prior arrangements have been made.
