Salt Creek Fitness & Rehab is an Outpatient Physical Therapy Clinic & 24/7 Fitness Center located in Osage City, Kansas. Our state-of-the-art facility provides a place for you to take control and prioritize your health. Whether it is rehabbing an old or new injury, post-surgical therapy, or just wanting to exercise and make your wellness a priority - we got you!

Our Physical Therapy clinic is connected to our Fitness Center, so our patients may use and take advantage of the facility that is offered to the community. Our Fitness Center is a 24/7 facility that offers different types of memberships and provides a secure, clean, and open area to exercise. The Fitness Center has a range of equipment - from free weights, ellipticals, treadmills, stationary bikes, Cybex strength machines, and more.

We are centrally located between Topeka, Ottawa, and Emporia, Kansas in Osage City, Kansas. We currently serve residents all over Osage County, Kansas and other neighboring communities.

Our Story

Salt Creek Fitness & Rehab was built in 2006 and opened for business in January 2007 with the idea to provide Osage County and surrounding areas a local place to further benefit any health, wellness and/or fitness needs. Since 2007, Salt Creek Fitness & Rehab has been providing outstanding and professional Physical Therapy services for our hometown and surrounding communities. We take pride in being the area's #1 Physical Therapy clinic and only 24/7 Fitness Center! This family owned and ran business is top-notch & has your best interest in mind. Meet The Team

Kevin is the owner of Salt Creek Fitness & Rehab and opened the business in 2007. He graduated from Washburn University in 1993 where he earned his degree and became a Certified PTA. With over 29 years in the medical field, Kevin has lots of Physical Therapy experience treating different injuries ranging from traumatic, work-related, sports-related, surgical, and just the typical "wear and tear". Aside from Physical Therapy, Kevin uses his passion for fitness/weight-lifting and has personally trained many individuals, including some collegiate athletes. He is a father to 3 children - Dalton, Kaitlin, and Carter; step-father to 3 children; and Papa to 7 grandkids. Outside of work, Kevin enjoys spending time with his wife, kids, grandkids, and dog - Max.

Born and raised in Osage City, Kaitlin spent most of her time at Salt Creek - joys of being the owner's daughter! She received her Bachelor of Science in Business Administration degree from ESU in 2017. After a few years of working elsewhere, Kaitlin decided to come back to the family business. She enjoys handling the business transactions and billing, but most of all loves the personal interactions & relationships with patients and fitness center members. She is excited and dedicated to the success and future of Salt Creek Fitness and Rehab. Outside of work, Kaitlin enjoys doing all of the "mama things" with her daughter - Delaney and spending time on the farm making memories with her husband - Dakota. Aside from being a farm uber, she enjoys watching KSU football. Go Cats!

Jordan earned his Bachelor's degree from Pittsburg State University in 2017 while on a 4 year athletic scholarship to throw javelin. He continued his education at the University of Kansas Medical Center where he received his Doctor of Physical Therapy degree in 2021. Jordan joined Salt Creek Rehab in 2021 directly after graduating & we are very excited to have him a part of our team! Jordan's favorite part of being a DPT here is being able to build relationships with every patient and helping regain their quality of life while getting them back to their jobs, hobbies, and/or sports. He enjoys doing Outpatient PT because each day is different & he is able to remain active. His future plans include becoming certified in Dry-Needling and IASTM. Raised in Colony, Kansas, Jordan married his high school sweetheart - Morgan and they have two children - Olivia and Owen.
