Home Health Services

Skilled nursing care involves trained professionals performing services that are needed temporarily due to an injury or illness. These services include observation and assessment of the patient's acute and/or chronic illness, assisting the patient in learning appropriate self-care techniques, skilled procedures such as wound care, intravenous therapy, and medication monitoring, and specific protocols for high-risk diagnoses such as congestive heart failure, sepsis, and pneumonia.

Visions Home Health patients who are currently enrolled with us are eligible for social services provided by a licensed Social Worker. Our social workers can assist patients and their families with community resource identification and referrals, short-term, intermediate, and long-term care planning, focusing on the patient's need for independence and control, information about and assistance in completing Advance Directives/Medical Power of Attorney, and honor patient/family wishes for their care.

Wound care nurses treat patients who have both acute and chronic wounds, including burns, pressure ulcers, and surgical incisions that have not healed. Our nurses not only support healing but also provide preventative care to ensure that infection does not set in and that other complications do not arise. They have extensive experience in wound care, and they use evidence-based practices and the latest technology to provide optimal care.

Physical therapy involves the provision of services to individuals and populations to develop, maintain, and restore maximum movement and functional ability throughout the lifespan. Our physical therapists provide therapeutic treatment in circumstances where movement, independence, and function are threatened by aging, injury, disease, or environmental factors. They work with patients to prevent the loss of mobility before it occurs, by developing fitness and wellness-oriented programs for a healthier and more active lifestyle.

Occupational therapy focuses on adapting the environment, modifying the task, teaching the skill, and educating the client and family in order to increase participation in and performance of daily activities, particularly those that are meaningful to the patient. Our occupational therapists work with patients to improve their functional abilities, and they use customized interventions to help patients achieve their goals.

Speech therapy specializes in the evaluations and treatment of communication, swallowing, and cognitive disorders. Our speech therapists provide comprehensive assessments and individualized treatment plans to help patients of all ages overcome communication challenges and improve their quality of life.

Home Care Services

Home care services provide a range of assistance, including home support services or a combination of both. These services include home care aides, housekeeping, companionship, and pediatrics. Our home care aides are trained to provide personal care and assistance with activities of daily living, such as bathing, dressing, and grooming. They can also provide medication reminders and help patients manage their medications safely.

Our housekeeping services include light housekeeping, laundry, and meal preparation. Our companionship services provide patients with companionship and social interaction, which can help reduce feelings of loneliness and isolation. Pediatric services are provided by our team of experienced and compassionate pediatric nurses, who can provide care for children with special needs or chronic illnesses.
