Nursing & Therapy Services

Our nursing and therapy services are designed to help our patients manage their acute and chronic conditions, prevent hospitalizations, and improve their overall health and well-being. Our home health nurses provide multidimensional care to patients of all ages, using evidence-based practices to achieve their health goals. Our therapy services include physical, occupational, and speech therapy, which can help our patients increase their physical activity, improve their lifestyle choices, and manage their symptoms.

Personal Assistant Services

Our personal assistant services (PAS) are designed to help our patients with everyday tasks, such as bathing, dressing, grooming, and meal preparation. These tasks are called activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Our personal care assistants are trained to provide compassionate, respectful, and dignified care to our patients, while helping them maintain their independence and quality of life.

Remote Patient Monitoring

Our remote patient monitoring (RPM) services use technology to enable monitoring of patients outside of conventional clinical settings, such as in the home or in a remote area. This can increase access to care and decrease healthcare delivery costs, while providing our patients with the support and care they need to manage their health.
