Customer Portal

Our customer portal is a one-stop resource for all of the important information you need regarding your healthcare plan. Here, you will find your policy documents, mobile apps and other tools, medical, travel, safety and cultural information, and details about how to access hospitals, physicians, clinics, and health and wellness specialists. If you have purchased our International Health & Wellbeing option, you will also have access to an online health program. Additionally, claim forms are available in the customer portal, and more details about claims can be found here.

Home Care Services

At Patients' Care First, we are committed to helping patients who are chronically ill or disabled achieve their health goals in the comfort and familiarity of their own homes. Our skilled services include:

• Skilled Nursing: Nursing services ordered by a physician for a homebound patient whose illness, injury, physical, or mental condition creates a need for the service.
• Skilled Therapy: Services performed by a physical, speech-language pathologist, or occupational therapist. These services are inherently complex and can only be safely and/or effectively performed by a skilled therapist.
• Home Health Aides (HHAs): Personal care services for our patients, provided under the supervision of a Registered Nurse or a Licensed Therapist. HHAs perform essential health-related services that are necessary to prevent or postpone institutionalization.
• Medical Social Worker Services: Long-term planning to promote independence and provide opportunities to take advantage of community resources.

Our skilled nursing services are provided by nursing staff who are trained to treat highly technological and medically fragile patients. Some of the services we offer include chronic diseases management and education, medication management, post-operative care, injectable treatment, high teach, IV infusion therapy, wound care, catheter care, venipuncture, and ostomy management.

Our skilled therapy services include physical therapy, speech therapy, and occupational therapy. Physical therapists specialize in restoring and maintaining maximum musculoskeletal function, whether it is a hip fracture, a sprain, or an unsteady gait. Occupational therapists strive to improve the level of independence in activities of daily living. Speech therapists assist in improving swallowing disorders and the communicative activities of daily living.

Our HHA services include bathing and grooming, dressing, activities of daily living, transfers, walking, medication reminders, meal preparation, and light housekeeping.

Medical social worker services facilitate long-term planning to promote independence and provide opportunities to take advantage of a variety of community resources designed to assist the elderly with their personal concerns and help alleviate their troubles.

Long Term Care Service (L.T.C) Personal Care Assistance Services (PCA)

PCA services are designed for people who need help with day-to-day activities. These services help people be more independent in their own homes and in the community.
