We are committed to providing specialized, coordinated, interdisciplinary, and evidence-based care approach to patients under the direction of a physician. Our mission is to promote the optimal patient well-being and to assist the patient in remaining at home, preventing hospitalization or admission to long-term care institutions.

Skilled Nursing

At Western Mass Home Health Services, we offer a wide range of skilled nursing services to meet the unique needs of our patients. Our registered nurses (RNs) and licensed practical nurses (LPNs) are highly trained and experienced in providing a variety of nursing services, including wound care, medication management, and disease management. We work closely with our patients and their physicians to develop individualized care plans that are tailored to the patient's specific needs and goals.

Non-Skilled Care

In addition to our skilled nursing services, we also offer non-skilled care to help our patients with activities of daily living (ADLs) and other tasks that may be difficult due to illness or injury. Our home health aides (HHAs) are trained to assist with a wide range of non-skilled services, including personal care, light housekeeping, and meal preparation. We work closely with our patients and their families to ensure that their needs are met and that they receive the support they need to live as independently as possible.

Rehabilitative Care

At Western Mass Home Health Services, we also offer rehabilitative care to help our patients recover from illness, injury, or surgery. Our team of physical therapists, occupational therapists, and speech therapists work closely with our patients to develop individualized treatment plans that are tailored to the patient's specific needs and goals. We are committed to helping our patients regain strength, mobility, and independence, so they can return to their normal activities as soon as possible.
