Community Home Health Services, LLC is a locally owned and operated business that has been proudly serving the Wyoming community since 2009. Our mission is to provide exceptional in-home health care services to patients in Hot Springs County, Washakie County, and Big Horn County. In-Home Care Services

At Community Home Health Services, LLC, we understand that every patient has unique needs and requirements. That's why we offer a wide range of options for at-home care to ensure that every patient is comfortable, safe, and happy in their own home. Our services include:

• Skilled Nursing Care: Our registered nurses work closely with your physician to develop a personalized plan of care for your medical needs. They assist with wound care, medication management, post-operative care, IV therapy, ostomy care, catheter management, lab draws, diabetic care, disease process education and management, and other skilled care that you may require.
• Home Health Aides: Our certified nursing assistants aid you with personal cares such as bathing, dressing, grooming, skincare, and other services needed to promote your recuperation and ability to remain at home.
• Occupational Therapy: Our certified therapists assist to maximize your independence with activities of daily living and self-care. They work closely with you and your physician to develop a personalized plan of treatment, including muscle reeducation, therapeutic exercise, home safety, adaptive equipment needs, and other treatments as you may require. Occupational therapists also specialize in work injuries and rehabilitation of the shoulder, elbow, wrist, and hand.
• Physical Therapy: Our certified therapists aid you toward regaining optimal physical health and range of motion. They work with you and your physician to establish a home exercise program and develop a personalized plan of care. Therapists provide therapeutic exercises following surgery or medical changes, fall prevention, balance and gait training, strengthening, endurance, and specific modalities you may require for optimal rehabilitation.

We work closely with each individual and family to put together a program of care that meets and exceeds your needs and expectations. Each program is coordinated with your doctor to ensure that you are getting exactly the care that you require. Our services include, but are not limited to, the following: personal care, IV therapy, pain management, nursing assessment, case management, transportation, skilled nursing, medication planning, education, wound care, diabetes care, personal care and hygiene, VA aide services, and all home health aide services.
