Skilled Nursing

Our skilled nursing team, comprising of Registered Nurses (RNs) and Licensed Practical/Vocational Nurses (LPN/LVNs), provides personalized care to our patients. Our skilled nursing team delivers high-quality care, including, but not limited to:

• Administering medications and treatments as prescribed by physicians
• Providing wound care and dressing changes
• Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Monitoring vital signs and health status

Physical Therapy

Our Physical Therapists (PTs) provide rehabilitation services to help patients recover from illnesses, injuries, or surgeries. Our Physical Therapy services include:

• Assessing and treating mobility limitations
• Increasing strength and range of motion
• Improving balance and coordination
• Providing education on proper body mechanics and exercises

Speech Therapy

Our Speech-Language Pathologists (SLPs) work with patients to improve communication skills, including:

• Assessing and treating communication disorders
• Improving language processing and comprehension
• Enhancing cognitive function and memory

Medical Social Services

Our Medical Social Workers provide emotional support and counseling to our patients and their families. Their services include:

• Assessing and addressing emotional and social needs
• Providing emotional support and counseling
• Assisting with advance care planning and hospice care

Home Health Aides

Our Home Health Aides (HHAs) provide supportive care and assistance with Activities of Daily Living (ADLs). Their services include:

• Assisting with bathing, dressing, and grooming
• Performing light housekeeping and laundry
• Assisting with meal preparation and feeding

Eligibility for Home Healthcare

You may be eligible for our home healthcare services if you are 65 or older, have been diagnosed with a new illness, have had your medication recently changed, have had your medical condition recently changed, require therapy services, would rather be cared for at home, have been hospitalized recently, or feel you're at risk for a fall.
