What We Do

We wrap our home health services around our patients to help them improve their health and wellness and remain in their home of choice. We work closely with a patient's current care team to develop a coordinated plan of care. Our services include Physical Therapy, which assists patients in regaining physical motion and strength, Occupational Therapy, which helps patients improve daily living skills, Speech Therapy, which addresses communication and swallowing difficulties, and Skilled Nursing, which provides advanced medical care. We serve patients within and outside of Americare's senior living communities, focusing on improving health and wellness wherever they call home.

Services

Inspire, Home Health by Americare, completes a comprehensive evaluation of each prospective patient in their own home, prior to discharge from hospital or inpatient rehabilitation, or in the assisted living setting. Our services are designed to address a range of conditions, including recent hospitalization, new medications, recent falls, unsteady gait, weight loss, exacerbation of chronic disease, new diagnosis, wounds, diabetes education, difficulties performing activities of daily living, difficulty speaking or swallowing, cognitive changes, increased weakness, and multiple doctor visits. We tailor our services to meet the unique needs of each patient, ensuring they receive the best possible care.
