Commonwealth Medical Physicians Group is conveniently located at 2800 N. Sheridan Road, Suite 400, Chicago, IL 60657. Our office is open Monday-Friday from 8:30 AM to 5:00 PM and Saturday from 8:30 AM to 1:30 PM. We are closed on major holidays including Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas, and New Year's.
