Referrals and New Clients
If you would like to make a referral or are a new client, please complete our referral form and send it to us at info@mysoulcare.org or fax it to 888-972-4998. You may also complete a new client registration form and give us a call at 980-613-8312 to schedule an intake and evaluation.
Authorization and Release of Information
We understand the importance of confidentiality and adhere to the Health Insurance Portability and Accountability Act (HIPAA) Privacy Policy. We require an Authorization to Release Information for insurance billing purposes and client communication.
Credit Card Payment Authorization and Payment
To make it easy for clients to access our services, we accept credit card payments.
