Our Story
Community Health Northwest Florida is a non-profit, Federally Qualified Health Center and Patient Centered Medical Home. We are driven to make healthcare accessible to everyone in Escambia County, Santa Rosa County, and surrounding areas, regardless of age, gender, race, religion, sexual identity or preference, circumstances or ability to pay. Our mission is to provide a full range of health and wellness services to all who walk through our doors.

Sliding Fee Scale
We understand that not everyone has the ability to pay for healthcare services. That's why we offer a sliding fee scale payment solution and flexible payment plans for those who qualify. Our sliding fee scale is designed to make healthcare more affordable for those who need it most.

We understand that having health insurance can be confusing. That's why we have a team of qualified healthcare professionals to help you navigate the process. We also offer a patient portal to help you manage your healthcare and communicate with your healthcare team.
