Office Information

Our office is conveniently located at 331 S. Moorpark Road, Thousand Oaks, CA 91361. We are open Monday to Thursday from 8:00 am to 5:00 pm and Friday by appointment. We accept cash, checks, MasterCard, and Visa. We also offer Care Credit for patients who prefer a payment plan.

Insurance and Billing

We are happy to bill all traditional type insurances and PPO plans. Please contact us for more information.

To request an appointment, please call our office or schedule online. We ask that you provide at least 48 hours' notice if you need to reschedule or cancel an appointment.
