Advocacy and Policy

Affiliated Partners Program

AzHHA's Affiliated Partners Program is designed to help healthcare organizations connect with trusted vendors and suppliers. Arizona POLST

The Arizona POLST (Physician Orders for Life-Sustaining Treatment) program is a collaborative effort between AzHHA and other healthcare organizations. The program is designed to help patients communicate their end-of-life care wishes to their healthcare providers. POLST forms are tailored to each patient's unique needs and preferences, and they ensure that patients receive the care that they want and deserve.
