Achieve Healthcare is a Minnesota-based PCA Choice and Traditional Agency established in 2001. We empower individuals with disabilities to live independently and with dignity in their own homes and communities. Achieve Healthcare is owned and operated by Dr. Jesse and Toni Ternus. We are a team of experienced professionals dedicated to providing exceptional care and fostering meaningful relationships with our clients. Our Services

We offer two primary service options:

1. Traditional PCA Services

• Assistance with daily living activities such as dressing, bathing, toileting, eating, and mobility.
• Medication reminders and management.
• Transportation assistance.

2. PCA Choice Program

This program empowers individuals to have greater control over their care by hiring, training, and managing their own Personal Care Assistants (PCAs). We act as a fiscal intermediary between you and your chosen PCAs.
