Meet Our Owner/Administrator

Rosa Yesenia Hernandez, RN, is the owner and administrator of Heart 2 Heart Concierge Home Care LLC. She has more than 30 years of medical experience and is committed to providing excellent care to every individual we serve.

Hernandez was raised by her grandparents in her native country of El Salvador. When she was 10, her grandmother became ill and passed away, a turning point in her life where she made a promise to help many others in her honor.

Hernandez migrated to the United States at the age of 12. She became a single parent of two, but this was not an obstacle to keep her from climbing the ladder. She graduated from James Lorenzo Institute in 1993, where she obtained her certified nurse assistant (CNA) certificate, and began to work at NCH. She attended night school to receive her licensed practical nurse (LPN) degree in 1995.
