Our mission is to help our patients recover from illness, injury, or surgery as quickly as possible, or to maintain maximum healthcare potential and the ability to stay in their own homes. Our Services

We offer a complete range of services in the home, with someone available 24/7 to answer your questions. We provide two different kinds of home care: skilled care and long-term care.

Skilled care can be any care at home that requires the services of a nursing or therapy professional. Our experienced nurses offer wound assessment and dressing care, disease monitoring and assessment, monitoring and dispensing medications, and educating the patient about disease management with the goal of self-care and independence. Our therapists assist with strength and balance issues, speech problems, injury rehabilitation, and teaching individuals how to become independent again by dressing themselves, maintaining personal hygiene, and eating.

Long-term care involves a home health aide assisting with personal care and housekeeping duties. Our aides provide companionship and assist with self-care and everyday tasks, while also providing companionship. We offer specialized programs to provide specialty care for patients with unique needs such as personal care/homemaking, progressive endurance, cardiac rehabilitation, wound care, pain management, medication management/teaching, diabetic management/teaching, home safety, and respiratory care/teaching.

To be eligible for home care services under Medicare, you must meet the following criteria: require intermittent skilled physical therapy, nursing care, or speech therapy; be under a physician's care; be homebound; and require a taxing effort and the use of assistive devices like a cane or wheelchair to safely leave home.

We are providers for Medicare, Medicaid, Passport, MyCare Ohio, Anthem, Aetna, Humana, The Health Plan, Summa Care, Caresource, United Health Care, Waiver, and many other private insurance companies.
