Our Services

At Home Health Connection, we offer a wide range of services tailored to meet the unique needs of our clients and their families. Our services include:

• Nursing Services: Our skilled nursing services include assessment, infusions, patient and family education, medication reconciliation, and wound care. We also provide shift nursing services for tracheotomy care, ventilator management, brain and spinal cord injuries, wound care, short bowel syndrome, pressure ulcer prevention, and orthopedic disorders.
• Therapy Services: Our therapy services include balance improvement, strengthening, mobility, gait, transfer education, physical therapy, occupational therapy, speech therapy, and social work. Our therapists work closely with our clients and their families to develop personalized care plans that promote healing, wellness, and independence.
• Caregiver Services: Our caregiver services include in-home companionship, personal care, 24/7 live-in care, bathing, grooming, dressing, meal preparation, light housekeeping, errand services, and laundry. Our caregivers are highly skilled, dedicated, and professional, and are committed to providing the highest level of care and support to our clients and their families.

Home Health Connection was started in 1999 by Shawn Mafi, who continues to lead the organization as a staple in the Home Health Care Community of Central Ohio. Our team is highly skilled and extremely empathetic, with a shared commitment to providing the highest level of care and support to our clients and their families.
