To request your medical records, please write to Maine Centers for Healthcare at P.O. Box 238, Scarborough, ME 04070. Kindly note that a fee of $45.00 is required to cover the processing, production of a CD, and mailing fees. Please allow 60-90 days for the completion of your request.
