Our Services

We offer a range of services designed to help individuals recover, manage their health, and maintain their independence. Our services include:

• Personal care home services, where we provide daily assistance and warm companionship to help clients feel safe, independent, and empowered in their personal care homes.
• Signature services, which include friendly companionship, skilled care, and adaptive care practices tailored to meet the unique needs and personalities of each client.
• Community residential alternatives, where we provide CRA services for people who require intense levels of residential support in a small group setting or in host homes/life sharing.
• Nursing services, including care co-management, medication reminders, and specialized care for dementia, Alzheimer's, and mental health.

Our Goal

Our goal is to help people live as independently as possible during recovery from a surgery, injury, or illness, managing a serious or chronic disease, or dealing with multiple diagnoses. We also work with you, your family, and your doctors to provide medication reminders and teach you about living on your own with the assistance of an aide. Our team helps you focus on living while we watch for warning signs, working with your family to treat potential problems before they turn into something serious.
