Insurance Coverage

AGADA Physical Therapy & Integrative Health Center is an in-network provider for most major insurance providers. Minnesota is a direct access state, which means that some insurance plans do not require a physician's referral. Please call your insurance company to see if your plan requires a referral or doctor's order. The contact information related to your plan should be on the back of your insurance card. We also accept cash-pay for physical therapy services.
