- We begin by conducting a non-medical assessment to determine the client's level of functioning, needs, desires, and conditions of client and the family in order to establish what services will be provided through discussion with the client and involved family members.
- We work in partnership with the family to develop an individualized program to manage the client's daily needs and match their needs to the best possible caregiver.
- We continually communicate with the caregiver who is providing care to the client and maintain our unique system of continued personalized contacts through telephone check-in or home visits.
