We provide comprehensive, patient-centered, coordinated primary care and integrated behavioral health services to maximize health outcomes, regardless of a patient's ability to pay. We welcome all in our community, regardless of insurance status, offering a sliding fee discount program for our uninsured or underinsured patients.
Our Services
Our team of dedicated healthcare professionals provides a wide range of services, including health promotion, chronic disease management, acute condition treatment, telemedicine, and HIV prevention and care. We also offer school-based clinics, providing medical care to students at no extra charge to the families. Our patient portal allows patients to access their medical records, schedule new appointments, and manage their payments online.
Our Impact
We are committed to providing low-cost, high-quality healthcare to our community. In 2021, we served over 13,350 patients across our two locations and 26 school-based clinics, providing over 125,000 patient visits. We also provided low-cost medications to over 1,200 patients through our Prescription Assistance Program. Our team of healthcare professionals speaks multiple languages, providing over 15,000 hours of patient service in languages other than English.
