Our Services

We offer a wide range of services designed to promote good health and wellness in a professional and caring environment. Our comprehensive medical care services include home visits, transitional care, chronic care management, prevention and wellness, health E&M @Home, X-rays and labs @ Home, companion CARE@HOME, and Care@Work (office visits). We also provide free blood pressure, weight, and oxygen saturation monitoring, as well as remote monitoring devices that allow you to track your health from home.

Our team of medical professionals is led by a Board Certified Medical Doctor who will oversee the wellness path for every patient, either in our office or at your home, or place of residence. We have a team of discreet and experienced doctors and certified nurse practitioners who make old-fashioned HOUSE CALLS to your home or residence.
