Urbandale Health Care Center is a 106-bed skilled nursing and rehabilitation facility that offers a full spectrum of post-hospitalization care, including Powerback Rehabilitation therapy services, short-term skilled care, and long-term care. We provide 24-hour medical care and support services to our residents. Our dedicated medical team provides a full spectrum of nursing and specialty services 24 hours a day. We believe in a holistic approach to care by meeting the needs of our residents physically, mentally, and socially. We do this by utilizing the knowledge and experience of our entire team, including therapy, nursing, dietary, activities, and social services.
